Neal,
Any driveshaft shop can make the coversion with off the shelf parts, I
have had two done.  I'm sure others can give more information.  As far
as the Ball and trunnion joint if it  is in good condition I would
service it and install a new boot.  If I had known boots and parts
were available I would not have coverted mine.
