I too feel badly about Gary's problem. His name is lifted in prayer at our church in San Jose.
I hope he's quit smoking as that apparently has a lot to do with bone healing.
Several years ago, while living in rural Illinois, I had a friend who broke his ankle.
They put hardware in his ankle to get it back together. He then developed a staph infection, later said to have been caused by the stainless steel screws into the bone.
He also had multiple surgeries, ultimately ending up
with removal of all hardware, followed a few months later by removal of his foot. In the end, which came a few years later, it did not end well for him.
I think infections in the foot like this are not
simple problems. I agree he ought to be at someplace like Mayo.

procedure doesn't work the first three times, what makes them think it will work
the fourth time? Why do these infections keep coming back? Seems to me it
is time for a second (or third) opinion.
Again, I realize I probably shouldn't be sticking my 2 cents in, but I
just hate to see Gary go through this, and cannot help but think there
must be a better way.

Gary Goers is one of the critical resources for our cars. If someone out
there with some free time is looking to do a good deed, maybe they'd be
willing to head up to Montana and give Gary and Alana a hand while Gary is
recouperating.
I hear that Kalispell is beautiful this time of year.
